  • Click on the Portal https://cp429.pharmcrm.com/ link
  • You will be redirected to Citizen Pharmacy’s portal.
  • Sign Up process for Provider Registration
  • Select the provider type from the dropdown menu.
  • Enter the Business Name.
  • Provide the Phone Number and NPI Number.
  • In the Provider Executive section, enter the Last Name, First Name, and Email ID.
  • Acknowledge the Privacy Policy and Terms of Use by ticking the checkbox.
  • Click on “Register.”
  • An OTP (One-Time Password) will be sent to the registered email ID.
  • Enter the OTP and reset your password to complete the registration process.
  • Log in using your email ID and password.
  • Enter your Last Name.
  • Enter your First Name.
  • Provide your Phone Number.
  • Select your Date of Birth by clicking on the calendar icon and choosing the
    appropriate date.
  • Enter your Email ID.
  • Select your Gender from the provided options.
  • Acknowledge the Privacy Policy and Terms of Use by ticking the checkbox.
  • Click on “Register.”
  • An OTP (One-Time Password) will be sent to the registered email ID.
  • Enter the OTP and reset your password to complete the registration process.
  • Log in using your email ID and password.
Note: Organizations do not need to sign up through the portal. They can directly
access their account by logging in.

There are two ways you can send us your prescription. You can either tell your doctor that you’d like your medication sent to Citizen Pharmacy or call us and give us your current pharmacy details to transfer your prescription to us.

We handle your refills and we will remind you as well so you won’t have to worry about remembering the dates. We work with your physician and insurance provider to ensure you always have drug access.
